• I hereby give my informed consent to undergo a CAA/ EASA Class 1,2 or 3 medical examination, (including substance abuse testing if required by the CAA/ EASA or any other licencing authorities). This will include testing for opioids, cannabinoids, amphetamines, cocaine, hallucinogens and sedative hypnotics e.g. benzodiazepine and to the electronic and written statutory disclosure of the results and outcome of all examination reports to the UK Civil Aviation Authority, EASA, and any other authorised body or medical practioner. I understand that during the examination a chaperone is available upon request. I consent to receiving electronic communications from Heathrow Medical Services LLP.
  • I confirm that I will fully disclose all my past and present medical information to the Aeromedical Examiner (AME) and understand that failure to do so may invalidate my fitness to work on board an aircraft. I agree that Heathrow Medical Services LLP and the examining AME will not be held liable for the costs of any further tests, investigations and specialist consultations that may be deemed necessary or subsequent change to an alternative EASA licencing authority and that I will therefore remain soley liable for all additional costs incurred, including any referral or processing and administrative fees as charged by Heathrow Medical Services.
  • I further agree that Heathrow Medical Services LLP will not be held liable for any additional costs or delays incurred should the medical examination be cancelled or postponed, if the relevant medical standards are not met, delayed receipt of specialist reports or as a result of the absence of the aeromedical examiner or optometrist due to illness, annual leave or other unforeseen circumstances. I understand that communication will be via e-mail via [email protected] for audit purposes. Should the case require deferral to the CAA/ EASA National Authority (NA) I understand that they may also require additional reports in addition to that already provided and that this may incur an additional expense. Any attempt to inhibit our regulatory functions may also be communicated to the relevant licencing authorities.
  • I am aware that there is an appeals process in place via the UK Civil Aviation Authority/ EASA (NA) and understand that if the CAA/ EASA (NA) communicate with me directly (or via the AME) I will be responsible for liaising with them from that point onwards and for paying any additional re-referral costs if this is requested by the CAA/ EASA (NA). Heathrow Medical Services LLP operate a zero tolerance policy with respect to threatening, abusive or intimidatory behaviour which will result in the authorities being notified.
  • I understand that I may view the report at any time in accordance with GDPR and Data Protection Act (2018) and that Heathrow Medical Services is a CAA/ EASA approved training centre and may train medical personnel accordingly who may be present during the examination.
