Approved medical practitioners
Anyone working on a seafaring vessel that will be travelling offshore is required by the MCA (Maritime and Coastguard Agency) to attend the ENG1 assessment before starting their role. The assessment checks that you are in the best physical condition to perform your offshore duties and also verifies that you have no medical conditions that could affect your safety or the safety of others.
As is the case with any required medical assessment, the ENG1 needs to be conducted by an approved medical practitioner who has a working understanding of the industry’s requirements. At Heathrow Medical, our MCA medicals are performed by our MCA-approved doctor who is also able to conduct a separate Chester Step Test if needed and advise you on any next steps and restrictions in the event of a ‘Fit with Restrictions’ certification.

Same day certification
Once all the relevant checks have been completed by our team you will receive your certificate on the day of your appointment which will certify you as either fit without restrictions or with restrictions.
A ‘With Restrictions’ certificate, or an outright ‘Unfit for Duty’ certification, means that your medical practitioner has identified a condition that will restrict your ability to perform your duties safely. If you do get one of these certificates, it’s important not to worry. Most conditions do not immediately result in a failed ENG1, and those that do are often subject to further testing. For example, if you fail the colour vision test then you must be declared unfit, but a CAD test will be able to confirm the type of colour vision loss that you have and its level of severity.

The ENG1 certification is valid for 2 years from the date of issue, or 1 year if you are under 18 years of age. We offer all of our appointments at a time that is convenient for you, so what can you expect on the day of your ENG1 medical?
Preparing for your examination
In order to correctly assess your medical history and any current conditions, we ask that you bring the following to your appointment:
- Identification in the form of your passport, driving licence, or seafarers book.
- Your latest ENG1 certificate if this is a renewal application.
- Details of any current or previous medical conditions and any medication you are taking.
During your examination
The ENG1 takes around 40 minutes to complete and certification is dependent upon compliance with all relevant MCA regulations. You will be required to fill in a questionnaire on your medical history and have a conversation with your practitioner regarding any current medical conditions that may affect your abilities at sea ahead of the examination, which will include the following:
- A vision test
- A hearing (audiometry) test
- A general physical examination to check your lungs, blood pressure, reflexes, etc.
- A measurement of your height and weight
- A urinalysis
Whilst this medical is a non-invasive assessment, please note that you will be asked to undress down to your underwear for certain tests during the physical examination.
